40 year- old Mukesh gets a new life from deceased donor liver transplantation

Kolkata, 31^st January, 2020: A new life unfolds for Mukesh Kumar Shaw, 40 years old resident of Nivedita Park Road,  Behala through a Cadaveric Liver Transplant done on 13^th of January at Medica Superspecialty Hospital, Kolkata.

Mukesh is the sole bread earner of his family comprising of his retired father and two sisters. His disease made the family worried about the future of their very existence.

Mukesh came to Dr. P.K Sethy, Director, Medica Institute of Gastrointestinal Diseases around the beginning of November by which time his liver was having an Acute Exacerbation of chronic end stage liver disease. Realizing the seriousness of the situation, Dr. Sethy advised investigations immediately as he foresaw the possibility of an Urgent Transplant to save this young man.

Dr. Sethy said, “We moved very fast on him from seeing him at OPD to planning for the transplant and listing him for the deceased donor’s liver as significant time already passed  when he consulted me.” After reviewing his initial reports, Dr. Sethy referred him to Dr. Hirak Pahari, Dr. Nikhil S & Dr. Barun Nath, Consultants, Department of Liver Transplant & Hepatobiliary Surgery. Pre-transplant work- up was done thereafter and necessary formalities were done with ROTTO for listing for a cadaveric live as he did not have a live donor available.

As per Dr. Hirak Pahari, “When we first saw Mukesh around early November, he was very sick with intermittent bleeding esophageal varices, severe jaundice, ascites & hepatic encephalopathy. His Na-MELD score was 31 which predicted a very high mortality risk of 30-40% in three months.” Mukesh was treated for acute on chronic liver failure until a suitable deceased donor became available.

The momentous time came on 13^th January this year when he underwent Deceased Donor Liver Transplantation (DDLT) from a brain dead donor. His surgery lasted exactly 5 hours 32 minutes and in the words of Dr. Hirak, “his post-operative period of twelve days at the Medica was uneventful.” Dr. Nikhil S was overwhelmed by the patient’s courage. In his words, “patient’s confidence of getting better further was noteworthy. He was always sure that he would get well via the transplant.”  Dr. Barun Nath said, “our planning was very meticulous and everything went as per that. The patient responded well post operatively and that made us discharge him on time as we had thought of.” The surgery had been ably supported by the Team of Anaesthesiologists headed by Dr. Amiya Mishra, Senior Consultant & HOD, Department of Anaesthesiology & Pain Medicine. “We have a highly skilled team of anaesthesiologists who are certified in Liver Transplant Anaesthesia who could successfully manage the liver transplant on the patient with gross liver induced bleeding disorder and compromised heart function”, told by Dr. Amiya Mishra.

After surgery patient was kept in the Liver Transplant Unit of Medica under the combined care of the Surgical Team and the Medica Critical Care Team. The critical care team developed a separate Transplant ICU and had a dedicated team that monitored and managed Mukesh 24x7 to minimize the risk of any post-operative infection and complications.

On 25^th January, Mukesh was discharged from Medica and the doctors guided him and his family to maintain the practices that were required to be followed for the patient’s safety and well being.

Mukesh was advised to be put up in a separate room and all his belongings must be thoroughly cleaned for his usage once he got back home. He was advised not to have too many visitors on his return. While interacting with outsiders or stepping out for a walk, he should be wearing masks, gloves & cap.

Now his dietary advice is normal with high protein intake. He is alerted not to lift any heavy weights for the next six months.

Post discharge Mukesh came to Medica on 27^th January and is found to be doing well. Dr. Hirak Pahari was happy to share, “the life in his eyes are back.”

While sharing his overall feeling, Mukesh told, “I was tensed initially, but always knew that I shall sail through as I had enormous trust on my doctors at Medica. During my stay every doctor I met were encouraging and that boosted my determination to get back to normal life as much as possible after the operation. I started walking with help from Physiotherapy Team in days after the surgery and with each passing day, I could feel that I was getting better. When I left Medica on 25^th, I was happy to return to my home but felt sad also as Medica became my second home during these days.”

The day witnessed the inaugural session of the 11th season of WWI’s acclaimed 5th Veda cultural hub. Inaugurating the occasion was the Queen of Indian Cinema, Ms. Madhuri Dixit-Nene. Following her introduction to the audience, the honoured guest joined Mr. Subhash Ghai, Founder & Chairman, WWI, Ms. Meghna Ghai Puri, President, WWI, and Mr. Ravi Gupta, Dean, for the traditional lamp-lighting ceremony.

The event began with an insightful AV that depicted the evolution of the 5th Veda cultural hub, followed by an AV highlighting Ms. Madhuri Dixit-Nene’s legendary career. Besides, she mentioned how yoga and meditation help in maintaining a balance between body and mind. As the evening progressed, she shared several interesting anecdotes from her movies. The context of budget 2020 is crucial and no doubt challenging ever before. Whether or not India will  able to hit the $5-trillion GDP goal within 5 years is one of the questions that currently dominating the concern among Indian. We have seen that GDP growth for 2019-20 is plunged to an 11-year low of 5%, primarily because of a poor show by construction and manufacturing. 

Companies in China, South Korea and Indonesia pay 25 per cent tax, while those in Malaysia pay 24 per cent. Only Japan has a higher tax rate than India at 30.6 per cent. Hong Kong has the lowest corporate tax rate of 16.5 per cent while Singapore has 17 per cent rate and Thailand and Vietnam levy 20 per cent tax on companies.

In recent year, India was the world’s fastest growing major economy. The past decade has been replete with predictions it would take up an increasing share of global commerce, alongside China and America. But the Philippines and Indonesia grew quicker than India last quarter and Malaysia was just a hair behind. China, grappling with its own slowdown, logged a respectable 6% and Vietnam was way ahead at 7.3%. 

So the expectations of budget 2020 goes many fold not only from Industry perspective also from rural Indian side. I would see the expectations overall from below areas more categorically:

In Corporates ,to give better execution the lengthy process of corporate M&As/ restructuring through the courts, the government should either prescribe a fixed time frame for the process or designate SEBI or department of company affairs as a single-window authority for all approvals. 

In health, Improve access to healthcare for low income households and Reduce health related costs for low income households

In taxation, real estate to include significant tax reliefs that can help prop up the sector.

In GST, some possible changes to facilitate small business to become more hassle free.

Considering the difficulties in proposing and implementing the budget 2020 at this juncture budgetary policy has to create  more trusting and rewarding environment for investors, keeping revenues stable and boosting demand through public investment and spending, and then India can expect a reasonable legacy in coming days.  

Custom Cup Regatta 2020

A Regatta Like No Other

White Sails and Vibrant Spinnakers dominated the Mumbai harbour skyline once again on 26^th January. The 2020 edition of the Customs Cup Regatta, one of the most anticipated yachting events in the Mumbai Harbour area, worked a thrill through even the most seasoned of sailors. The never seen before turnout of over 70 yachts was greeted by strong afternoon winds and salt spray at the race venue off the Gateway of India. Despite the challenge the wind threw up , the sailors were raring to go just as the race was flagged off by Additional Director General Vijay Digambar Chafekar, PTM TM, Coast Guard Commander (Western Seabird) and Commodore Bombay Customs Yacht Club and Principal Commissioner Smt. Rama Mathew.

The Customs Cup Regatta is a mixed class race for wind sailing boats held every year on 26^th January in commemoration of the International Customs Day. The Indian Customs, as a part of the Global Customs fraternity, has dedicated the year 2020 to Customs fostering Sustainability for People, Prosperity and the Planet. It is noteworthy that the Indian customs is dedicated towards implementing green customs and sustainable practices. True to the spirit of fraternity that the 26^th January embodies, the event hosted by Bombay Customs Yacht Club witnessed the turnout of yachtsmen from all over India like – Army Yachting Node, Electrical and Mechanical Engineers , Corps of Military Engineers, INWTC, Royal Bombay Yacht Club, Bombay Sailing Association, Colaba Sailing Club, Goa Yachting Association and other premier yacht clubs. BCYC was ably assisted by the RBYC, International jurists Major General SM Pillai, Colonel Sathish Kumar Kunwar and qualified international race officer Shri T. Srikanth Chaturvedi. The Regatta 2020 was sponsored by New India Assurance and IOCL.

The challenging race conditions of the afternoon tested the wits of every sailor. The bravado of competing sailors and their adept handling of yachts left every spectator gathered on ferries and boats around the race area awestruck. The race proceeded smoothly under the watchful and reassuring presence of the Indian Coast guard.

The coveted Customs Cup was won for the third time by defending champions Shri Prasad Shelar, Shri Prathamesh Bagwe, and Smt. Yugandhara Patil sailing the seabird ‘Neil’. The team also took home the Philip Bragg (seabird handicap) trophy.

The Bombay Customs Yacht Club has grown from strength to strength since the day of its revival by former Chief Commissioner Shri Bani Bhattacharya. The handholding by adventure sailor & Arjuna Awardee, Padmashri Gulshan Rai has laid the keel for another generation of seafaring Customs officers.

Indian Customs as the guardians of economic frontiers have a pivotal role to play in ensuring the security of a 7400 km long coastline. Scrap Tyres Re-Used as Dog Beds

Bahadurgarh, Haryana, January 31st 2020:  Yokohama India Pvt. Ltd, a fully owned subsidiary of Yokohama Rubber Co., Ltd., donated around 190 factory scrap tyres to Neighbourhood Woof - an NGO, in order to provide durable and comfortable beds for stray dogs in the shelter.

Neighbourhood Woof was started by Ayesha Christina, helps local stray dogs which got injured or had an accident also drives sterilization and vaccination programs. “For 6 years we struggled to find a sustainable winter arrangement for our dogs that could be washed, disinfected and re-used. Pre-budget Expectations from Mr. Kishore Jain, President CREDAI Bengaluru

The budget announced for 2019-20 glorified affordable housing more than offering few taxing solutions for the home buyers. Long-term expectations like industry recognition for the sector, single-window clearance were not been addressed and discussed in the last budget. That is why, this year, most of the real estate developers are expecting that these important factors must be addressed so that the sector can reinvigorate. Even though the announcement of INR 25,000 crore has given some relief to the sector, homebuyers’ growing interest to buy a project will be a major boost to the sector.

Tax Deductions

Hon’ble Finance Minister Nirmala Sitharaman, during her first maiden budget, announced an increase of INR 3.5 lakh tax deduction cap on the interest paid on home loans approved for the acquisition of the first home purchase up to Rs 45 Lakh in the financial year. It is good that the government is promoting affordable housing to fulfil the promise of “Housing for All by 2022” under PMAY scheme. But the missing factor is the benefit for the middle-class population. The Government did not address the housing concerns of the mid-segment population apart from strengthening RERA. Therefore, developers are looking at tax and interest benefits for the mid-segment which will help in increasing the sales.

Reduction in GST rates on construction materials

Construction material cost is directly dependent on the cost of raw materials and procurement charges. Although the government has reduced GST rates for the affordable and under-construction buildings, the materials like cement (28%), sanitary items (18%), steel, doors, windows, electrical continue to pose a challenge due to the high GST rates. The industry requests the government to reduce the inputs and input services tax to 5% - 12%.

RERA to consider multiple tower projects as an ongoing venture till the completion

In an integrated township project, until the construction of all the towers gets completed, RERA should consider it as the ongoing project. This will help in maintaining the GST rate of under-construction projects.
